1. Assist patients with personal hygiene, including bathing, showering, grooming (hair and nail care), and oral hygiene.
2. Help patients with dressing and undressing.
3. Assist patients with toileting needs, including the use of bedpans, urinals, and assisting to the toilet.
4. Provide skin care and pressure sore prevention (e.g., turning and repositioning patients, performing back care).
5. Assist with feeding patients, ensuring they receive appropriate meals and adequate hydration (excluding tube feeding).
6. Respond promptly to patient call bells and inquiries, determining their needs and reporting to the nurse as necessary.
7. Safely assist patients with ambulation (walking), transferring them in and out of bed, to wheelchairs, and to trolleys, ensuring correct body mechanics.
8. Measure and record vital signs (temperature, pulse, respiration, blood pressure) accurately.
9. Monitor and record patient intake and output (fluid balance).
10. Observe and report any changes in the patient's condition, behavior, or discomfort immediately to the supervising nurse.
11. Report any faulty equipment or instruments to the registered nurse.
1. Minimum Sijil Pelajaran Malaysia (SPM) or equivalent.
2. Preferable with Certificate in Basic Care for Healthcare.
3. Experience working in hospital setting is an advantage.
